The Fisher Original Astronaut Space Pen is 5 inches overall with a pocket clip and a PR4 black medium point cartridge.

The cartridge is pressurized with nitrogen rather than relying on gravity, so it writes upside down, at any angle, under water, over grease and in extreme temperatures. That was the point of the original design. A conventional ballpoint stops working the moment it is held above the horizontal, and in zero gravity it never works at all.

Fisher developed the pen independently and NASA adopted it for Apollo missions, and the same cartridge design has been in production since.
